TCCC Student Program

 

ARE YOU IN NEED OF A PAIR OF GLASSES, BUT CAN’T AFFORD THEM?

DO YOU ATTEND TARRANT COUNTY COMMUNITY COLLEGE?

If so, you may be eligible to receive a free basic eye examination and glasses.

To apply, please follow the steps below:

  1. Verify you meet all eligibility requirements outlined on this page
  2. Print copy of application – TCCC APPLICATION FOR VISION ASSISTANCE
  3. Return completed copy to Trinity River Campus CE office by September 13, 2013

If you do not have a FASFA on file with the Tarrant County Community College for the 2013-2014 school year, copies of household income documents must be attached to application for processing.

SERVICES ARE LIMITED and APPLICATIONS WILL BE PROCESSED ON A FIRST COME FIRST SERVE BASIS.

DEADLINE FOR APPLICATION SUBMISSION IS SEPTEMBER 13, 2013

 

ELIGIBILITY REQUIREMENTS

1.  Have no private or government insurance, including Medicare or Medicaid

2.  Have income below the established level based on household size — Income Level Chart

3.  Have not had an eye exam within the past 24 months

4.  Must be a US citizen or legal resident with a social security or legal resident number

5.  Have not received a doctor referral through the VISION USA program in the last two years

6.  Must be a student enrolled at Tarrant County Community College

NO EXCEPTIONS WILL BE MADE

A contact lens exam and / or contacts are not available through this program.

 

HOW THE PROGRAM WORKS

VISION USA provides free eye exams to eligible, low-income individuals. Services are donated by volunteer optometrists who are members of the American Optometric Association. If you are eligible to receive services, you will be matched with a volunteer doctor of optometry who will provide a comprehensive eye exam and glasses at no charge. The process is as follows:

  • Verify you meet all eligibility requirements outlined on this page
  • Print copy of application – TCCC APPLICATION FOR VISION ASSISTANCE
  • Return completed copy to Trinity River Campus CE office by September 13, 2013
  • Early to Mid October you will receive an email notification with the name of a volunteer doctor for you to contact directly to schedule an appointment for a no cost eye examination.  A contact lens exam and / or contacts are not available through this program.
  • Call the doctor within 30 days from the date of the email to schedule an appointment. Be sure to let the doctor’s office know you were given the doctor’s name through the VISION USA program.
  • Bring a copy of the email with you to your appointment.
  • Upon completion of your appointment a post-examination form is to be completed.
  • You are responsible for your own transportation and must be on time for your appointment to avoid scheduling conflicts.
  • Contact the doctor’s office 48 hours in advance of the day of your appointment should you need to cancel or reschedule.
  • Missed appointments will not be rescheduled and will further disqualify you from receiving free eye care. Appointment days and times are limited.
